About the Role
G&A Counseling is expanding our Targeted Case Management program and seeking dedicated case managers to join our team. You will work in the community connecting clients with medical, social, and educational resources.
Responsibilities
- Conduct community-based client visits
- Connect clients with medical, social, educational, and vocational resources
- Develop and maintain individualized care plans
- Coordinate with therapists, schools, and outside agencies
- Document all contacts and services in TherapyNotes
Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Valid Kentucky driver's license and reliable transportation
- Experience in community-based mental health preferred
- Strong organizational and time management skills
- Familiarity with Medicaid TCM billing preferred
