What is Targeted Case Management?

Targeted Case Management (TCM) is a community-based support service that helps individuals access medical, social, educational, and other essential resources. Our dedicated case managers work with you in your home, school, and community, meeting you where you are to ensure you get the support you need.

G&A's TCM program is led by Mary Alysia Foster, M.Ed., LPCC, C-PD, NCC. Our dedicated case management team is one of the largest TCM programs in the region, connecting clients with the resources they need.

The process includes comprehensive assessment, individualized care plan development, referral and linkage to services, and ongoing monitoring and follow-up to make sure you're getting what you need.

Who Is This For?

TCM services are available for:

  • Individuals with serious mental illness (SMI)
  • Children with severe emotional disturbance (SED)
  • Individuals with substance use disorders (SUD)
  • Those with co-occurring disorders (COD)
  • Anyone who needs help navigating the healthcare and social services system

What to Expect

Your case manager will meet with you regularly in your home, at school, or in the community to assess your needs, develop goals, connect you to resources, and follow up to make sure those services are working. TCM is about making the system work for you, not the other way around.

Services can be delivered in person or via telehealth. Referrals can come from healthcare providers, schools, courts, social services, or families.
